SMCF offers long and short term compassionate care in a small community setting. Our interdisciplinary team is professionally trained to provide complex medical support as well as individualized personal care.
Our approach provides an ideal solution for residents needing assistance with medications, bathing, dressing and other activities of daily living. Independence, wellness and maintenance of residents own routine is encouraged by an individualized plan of care.
